Solution
Forms don't really do too much unless they connect to what is called CGI (Common Gateway Interface). This sounds technical, but it's not. CGI is really just a fancy name for a script (a small program) that does a particular task. You can download many scripts for many purposes for many purposes from places like
www.hotscripts.com
, but for the task you require (sending a form to an email address) we would recommend a script called FormMail, which you can obtain from
http://nms-cgi.sourceforge.net/scripts.shtml
Make sure that you properly read the README file that is included in the zip file for FormMail, and you will be able to easily pop it into your cgi-bin directory and modify your web page to suit.
FormMail also requires to be CHMOD'ed to 755 (the permissions need to be changed to rwx,rx,rx) for the script to run. Please read the article called 'How do I change permissions for my scripts' if you need help here.
